COVID-19 Vaccine Clinic

Granite Park has partnered with Passport Health to host a COVID-19 Vaccine Clinic on Friday. This event is exclusively offered to Granite Park Customers only.
The clinic will be held in the Granite Park 1 Conference Room (5800 Granite Parkway, Suite 160B) on Friday, May 21 from 12:00PM-3:00PM. Passport Health will be distributing the Moderna vaccine.
Please read the information below before signing up.
- Please bring a PHOTOCOPY valid license and your insurance
- If you do not have insurance, still please bring your license
- Please print out and sign this consent form before arriving
- You must be 18 years or older to receive the vaccine
- There is no charge for the vaccine
- Please wear a face covering
- No other vaccines can be received 14 days before or after a COVID-19 vaccine
- Space is limited and registration is strongly encouraged (walk-ins may not be accepted)
