We are excited to welcome Passport Health to host an on-site vaccine clinic right here at Granite Park. The Flu Shot and the Moderna COVID-19 vaccine will be offered at the clinic. Passport Health will be visiting the Granite Park 1 and Granite Park 3 Conference Rooms. Sign up here for your spot today!
Please bring a printed photocopy of your insurance card and your license! There will not be a printer available at the clinic!
